Description
Total Testosterone measures the overall amount of testosterone circulating in the blood. In men, it’s a foundational marker for androgen status—but it’s not the whole story, because total testosterone includes hormone that is tightly bound to SHBG and not readily available to tissues.
Why it matters
Testosterone influences energy, libido, mood, red blood cell production, muscle mass, and bone density. Persistently low testosterone can contribute to symptoms and adverse body-composition trends, while very high levels may reflect supplementation or endocrine disorders.
How to interpret it well
Timing matters: levels are typically highest in the morning; clinicians often prefer early-morning testing.
Repeat to confirm: a single low value is often rechecked under typical sleep/stress conditions.
Check SHBG/free testosterone: if SHBG is high or low, total testosterone may not reflect tissue-level androgen availability.
Common drivers of low total testosterone
Poor sleep, obesity/insulin resistance, chronic illness, high alcohol intake, opioids, and aging can all contribute. Overtraining and caloric restriction can also suppress testosterone in some contexts.
